Community Care Network / Rutland Mental Health Services is Hiring a Manager of Service Coordination Near Rutland, VT

Community Care Network is looking for a Manager of Service Coordination to join our team!Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service.
About the Role:The Manager of Service Coordination is responsible for the leadership, planning, administration, and oversight of the DS Family Managed Respite Program, Flexible Family Funding Program, as well as overseeing general supports for individuals receiving home and community-based services (HCBS) Shared Living, and fee for services (FFS). This position provides supervision and ongoing support to direct reports.
Principal Responsibilities:
  • Planning, development, and management of services for individuals served through the HCBS, Shared Living, and FFS (i.e., targeted case management, specialized services in a nursing facility).
  • Assures the timely development and implementation of Individual Support Agreements and other required documentation by service coordinators including funding proposals.
  • Oversees service coordinators and provide ongoing monitoring of service quality.
  • Ensures compliance with all applicable agency, state, federal, guidelines, policies, and regulations.
  • Assists in budget development, quality assurance, and compliance activities.
  • Ensures that all programs operate within specified budget guidelines.
  • Assigns and monitors caseloads.
  • Participates in the overall planning, development, and evaluation of services including the Agency's strategic plan, DS outcome measures, and the Local System of Care Plan.
  • Provides clear expectations, ongoing support, feedback, and supervision to direct reports.
  • Supports service coordinators to resolve conflicts and problem-solve with teams, staff, individuals, guardians.
Qualifications:
  • Bachelor's degree in related field preferred; equivalent experience may be considered.
  • 4 years minimum experience in developmental disabilities services preferred.
  • Previous experience working in child and family services preferred.
  • Eligible for Qualified Developmental Disabilities Professional (QDDP) status.
  • Previous leadership or management experience required.
  • Strong leadership skills (i.e. accountability, follow through, coaching techniques, constructive feedback, multitasking…etc.).
  • Proficiency with computers, experience entering data into electronic medical records (EMR) program.
  • Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
  • Ability to respond in emergency situations appropriately.
  • Commitment to person centered services, community inclusion and individual choice.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.
