Title: Crisis Triage and Stabilization Supervisor

Reports to: Executive Director of Collaborative Bridges (Patrick Dombrowski, LCSW)

Position Type: Full Time

Salary: $70,000 a year

Purpose:

The Crisis Triage and Stabilization Supervisor will assist in developing C4’s Cook County Crisis Triage and Stabilization Program at the shared Collaborative Bridges clinical site in West Garfield Park at 4223 W Lake Street. They will oversee day-to-day operations and provide clinical consultation for individuals that present to Triage Center needing evaluation and assessment and will utilize their expertise in crisis care to ensure least-restrictive outcomes for all individuals served. The Supervisor will be responsible for direct oversight of Crisis Stabilization Specialists, including ongoing supervisor and oversight, as well as, training and development of best practice clinical skillsets. The Supervisor is accountable for ensuring the delivery of high quality comprehensive and cost-effective clinical services and enhancing overall service performance in a culturally sensitive and appropriate manner, while meeting the quality metrics established in concert with Cook County.

In addition, the Supervisor will participate in system coordination activity with Cook County and West Side Crisis Continuum community stakeholders. They will be responsible with marketing and communicating availability of services through outreach activities to West Side community. As part of the continuum of care services at shared Collaborative Bridges Wellness Center, they will participate in ensuring indiviudals served are connected to ongoing treatment and that Social Determinant of Health needs are assessed and linked as appropriate.

Duties and Responsibilities

  • Oversees the effective management and efficient integration of service delivery functions including, continuity, consistency, efficiency, and effectiveness of service delivery
  • Develops, implements, evaluates, and ensures compliance with service delivery operating policies, procedures, and reporting requirements.
  • Provides clinical supervision and guidance to community-based team comprised of multi-disciplinary skill sets.
  • Nurture a team philosophy to treatment and develop a culture of support for staff.
  • Familiarity with the population needs and resources of Chicago’s Westside.
  • Comfortability in provision of community-based services and understanding of basic safety protocol for staff and consumers in provision of community-based services.
  • Ensure effective transitions of care for individuals served.
  • Maintain effective communication and coordination with partners in Collaborative Bridges and great West Side stakeholders.
  • Assist with the creation and advocacy related to the short-and long-range service delivery operating strategies and is accountable for the implementation of such strategies in all crisis prevention, short term treatment, and community engagement related service areas
  • Develops, researches, promotes, and participates in training opportunities applicable to identified team members and for the clinical enhancement of identified teams and program goals
  • Initiates, cultivates, and maintains professional relationships within C4 and Collaborative Bridges to enhance communications, shared vision, support, and program alignment
  • Determines individual performance expectations for direct reports.
  • Fosters a collegial and transparent operating culture among other service providers at Wellness Center and within C4.
  • Initiates, cultivates, and maintains professional relationships with human services and government agencies, health service vendors, and public and private groups to enhance service delivery and Agency presence in the community
  • Works closely with leadership and clinical directors to evaluate systems for process improvements and efficiencies
  • Develops long range objectives and plans
  • Adherence to confidentiality practices and procedures
  • Performs other duties as assigned by Director of Program.

Competencies (Minimum Skills, Knowledge, and Experience):

  • Cultural humility and ability to build trusting relationships with consumers.
  • Detailed and extensive knowledge of crisis center behavioral health operational management and proven management skills including evaluation of clinical efficiency and effectiveness
  • Familiarity with Westside of Chicago.
  • Understanding of risk assessment and crisis stabilization evidenced based practices.
  • Ability to work on multidisciplinary teams.
  • Detailed and current knowledge of behavioral health principles and practices, with an emphasis on trauma informed care and intervention
  • Detailed and current knowledge of substance abuse principles and practices, with an emphasis on harm reduction philosophies and brief interventions.
  • Excellent interpersonal, verbal and written communication skills
  • Ability to plan, develop and manage multiple projects
  • Excellent written and oral skills to communicate effectively with all levels of staff, governmental and community representatives, and vendors
  • Ability to work within a team
  • Demonstrated cultural sensitivity and competency
  • Ability to present a positive image of Collaborative agencies to clients, candidates, stakeholders, and visitors
  • Ability to resolve issues objectively and maintain strict confidentiality
  • Ability to develop solutions to problems and to analyze qualitative and quantitative data
  • Knowledge of HIPAA privacy and security regulations

Qualifications


Minimum Qualifications:

  • Graduate Degree in Behavioral Health field.
  • LCSW, LCPC, or LMFT
  • In depth experience in crisis triage and stabilization treatment.
  • Superior leadership experience including direct staff management (i.e., feedback, coaching, and developing staff)
  • Bilingual skills (English/Spanish) are helpful but not required.
