Description

C4 seeks to equip 7,000 individuals annually to help them move from trauma to living, to working and thriving within their community.

Philosophy - C4 believes that people recovering from mental illness and emotional trauma are able to live, work and thrive in the community. 

Purpose:

The Resource Coordinator is responsible for enhancing care coordination, removing service barriers, and addressing medical and social needs of children, up to 21 years of age, living in the Austin and Belmont Cragin communities through comprehensive health screenings. They are accountable for ensuring the delivery of high quality and comprehensive services in a culturally sensitive and appropriate manner.

Duties and Responsibilities

  • Conduct annual functional needs assessment with families.
  • Become familiar and gain a working knowledge of Health Insurance Portability Accountability ACT (HIPAA).
  • Becoming familiar and gain a working knowledge of mandated reporting.
  • Ability to accurately document information in a timely manner.
  • Provide psychoeducation to families about mental and physical health needs.
  • Become familiar with organizations within the Austin and Belmont-Cragin communities and act as a liaison for those organizations to assist them with getting their families to complete the annual needs assessment or for the organizations to make referrals for youth with emerging needs.
  • Provide short-term (1-6 months) care coordination services for youth and their families.
  • Provide navigation and warm linkage to resources that address health, housing, vocations, mental health, substance misuse, food insecurity, education deficits, disability needs and other resources.
  • Conduct in-depth functional assessments for youth and their family identified as having higher needs.
  • Develop brief care plans to address needs identified through assessments
  • Meet face-to-face or virtually with families when indicated, at the office or in their homes
  • Connect families with needed services including helping families’ complete applications and periodically attend appointments/meetings with them to assist them in obtaining needed services
  • Interface with Medicaid Managed Care Organizations (MCOs) to coordinate care
  • Frequently make contact to assess a family's progress in meeting goals identified on the care plan.
  • Document care plans, contacts, and interventions in Electronic Medical Records system
  • Collaborate with other organizations serving youth and coordinate services amongst organizations to prevent duplication of services or gaps in care.
  • Initiate, cultivate and maintain professional relationships with human services and government agencies, health service vendors, and public and private groups to enhance service delivery and C4’s presence in the community.

  • Ability to present a positive image of the agency to clients and staff.
  • Ability to work under stress and successfully handle multiple tasks and projects simultaneously.
  • Ability to resolve issues objectively and maintain strict confidentiality.
  • Ability to deal with ambiguity and changing organizational needs.

Qualifications


Minimum Qualifications:

  • A high school diploma or equivalent and preferably training beyond high school such as training/certificate as a Community Health Worker.
  • Familiarity with computer operating systems.
  • Familiar with and knowledgeable of the communities in the 60651 and 60639 Chicago zip codes.
  • Personal vehicle, valid driver’s license, insurable driving record and vehicle insurance.
  • Experience working in healthcare or social service agencies preferred.
