As a member of the Youth and Family Services Outpatient Team, the therapist will provide office-based and telehealth services, including family and individual therapy to youth, families, and adults. Services consists of therapeutic interventions that facilitate healing, skill-building, identification and the use of natural supports, and the use of community resources. Services will be provided mostly at the agency, with ability to work from home for telehealth appointments. Must be available two evenings a week until 8:00 p.m.

Duties and Responsibilities:
1. Provide individual and family therapy and case management services
2. Assess clients functioning, strengths, needs, and diagnosis
3. Assess for indicators of abuse
4. Develop treatment plans collaboratively with clients by defining goals, specific outcomes, and time frames for treatment
5. Apply appropriate trauma-informed interventions
6. Provide culturally appropriate, competent, and individualized linkage plans in accordance with each clients age, gender identity, sexual orientation, race, ethnicity, religious beliefs, and culture
7. Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association forCommunity Psychiatry
8. Identify need for crisis intervention when indicated
9. Develop mental health assessments resulting in the identification of the individual's mental health service needs and recommendations for service delivery
10. Work cooperatively with other members of the treatment team
11. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
12. Assist with outcome measures
13. Maintain compliance with clinical documentation, timelines and agency policies and procedures,including meeting required timeframes for documentation and service notes
14. Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
15. Develop close working relationships with DCFS and other mental health service providers throughout the northwest and west sides of Chicago
16. Maintain compliance with confidentiality policies and procedures
17. Other duties as assigned by supervisor
