Job Details

Job Location: C4 NORTH - CHICAGO, IL
Salary Range: Undisclosed

Description

As a member of the Youth and Family Services Outpatient Team, the therapist will provide office-based and telehealth services, including family and individual therapy to youth, families, and adults. Services consists of therapeutic interventions that facilitate healing, skill-building, identification and the use of natural supports, and the use of community resources. Services will be provided mostly at the agency, with ability to work from home for telehealth appointments. Must be available two evenings a week until 8:00 p.m.

Duties and Responsibilities:

1. Provide individual and family therapy and case management services

2. Assess clients functioning, strengths, needs, and diagnosis

3. Assess for indicators of abuse

4. Develop treatment plans collaboratively with clients by defining goals, specific outcomes, and time frames for treatment

5. Apply appropriate trauma-informed interventions

6. Provide culturally appropriate, competent, and individualized linkage plans in accordance with each clients age, gender identity, sexual orientation, race, ethnicity, religious beliefs, and culture

7. Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association forCommunity Psychiatry

8. Identify need for crisis intervention when indicated

9. Develop mental health assessments resulting in the identification of the individual's mental health service needs and recommendations for service delivery

10. Work cooperatively with other members of the treatment team

11. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community

12. Assist with outcome measures

13. Maintain compliance with clinical documentation, timelines and agency policies and procedures,including meeting required timeframes for documentation and service notes

14. Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor

15. Develop close working relationships with DCFS and other mental health service providers throughout the northwest and west sides of Chicago

16. Maintain compliance with confidentiality policies and procedures

17. Other duties as assigned by supervisor

Qualifications


Minimum Qualifications:

  1. Masters degree in social work, psychology, counseling, or a related field is required
  2. One or more years of experience working with children and adults with mental illnesses is required. This can include internship experiences.
  3. Excellent interpersonal, verbal, and written communication skills

  2. Experience and high level of interest in working with youth, families, and adults
  3. Knowledge and experience with integrating mental health and physical healthcare
  4. Ability to use trauma-informed and evidence-based interventions
  5. Understanding and use of personal and professional boundaries
  6. Excellent time management skills, as some services will be provided at various locations in the community
  7. Excellent organizational skills as completing documentation in a timely manner is required
  8. Teamwork and collaboration skills
  9. Conflict-resolution and diplomacy skills
  10. Problem-solving skills
  11. Ability to demonstrate flexibility to meet clients and agencys needs

Duties and Responsibilities:

  1. Provide comprehensive mental health assessments, treatment planning, therapeutic services, case management, and treatment evaluation for youth and adults with a mental illness in person and via telehealth
  2. Facilitate treatment interventions to maintain or enhance the clients capacity for healing and a return to a person-centered baseline of functioning
  3. Provide services in the clients home, or in the community near the clients home, to overcome obstacles to the clients access to services, and to support the client in using community resources as fully as possible
  4. Interview and evaluate clients
  5. Develop treatment plans collaboratively with the client by defining goals, specific outcomes, and time frames for treatment
  6. Complete all necessary documentation (Mental Health Assessments, Treatment Plans, Progress Notes, etc.) in a complete and timely fashion, meeting agency standards/requirements
  7. Work collaboratively with the Clinical Supervisor and other members of the Children Outpatient team, as well as other community service providers, to coordinate and integrate all aspects of care
  8. Adhere to confidentiality practices and procedures according to agency policies
  9. Always maintain compliance with agency policies and procedures
  10. Meet direct service hour expectations for the benefit of clients
  11. Other duties as assigned by supervisor
