- The purpose of the CNA – Case Management position is to improve health outcomes through coordinating care by enhancing communication and the continuity of care. The Certified Nursing Assistant will consult with other healthcare team members to coordinate services for Community Health Center patients.
- The Certified Nursing Assistant will follow up on assigned patients' gaps in care, emergency room visits, hospital discharges, document in the Case Manager’s Bucket, and preventive care.
- Must live in Central Florida. The position is remote after training in Winter Garden for 30 days.
- High School Diploma or equivalent.
- Certified Nursing Assistant program approved by the Florida Board of Nursing.
- A minimum of 1 year of experience working as a Certified Nursing Assistant is required.
- Experience with Electronic Medical Records.
- Experience with Microsoft Office programs.
- Certified Nursing Assistant license with the State of Florida.
- Current Basic Life Support (BLS) certification.
- Must be completed within 30 days of hire.
- Maintains a professional relationship and positive attitude with co-workers, the public, patients, and the entire organization.
- Maintains the highest professional and work ethics.
- Displays enthusiasm toward the work, mission, and vision of the organization.
- Ability to use basic mathematical skills.
- Basic understanding of medical terminology and documentation.
- Able to perform repetitive tasks.
- Able to work flexible hours and sites as needed.
- Able to work evenings.
- Exceptional Customer Service skills.
- Ability to demonstrate organizational skills and attention to detail and possess a good working knowledge of electronic medical records.
- Ability to communicate effectively, verbally and in writing.
- Excellent grammar, spelling, and interpersonal skills.
