Just Wheeling Along - Caregiver

Do you want to take the ride? A Wisconsin Rapids female is looking for a backup/fill-in personal care assistance to keep her wheels rolling along as she continues to raise funds for our community and surrounding areas. Would you like to give back to those who give to others? Community Horizons is assisting with recruiting the perfect care provider to assist with this member's care so she can continue her work in the community and maintain her independence. Pay Rate: $15.00 / Hour

Part-Time: No set hours but looking for someone with the flexibility to assist with pick-up shifts needing coverage due to vacations, sickness, etc.

Tasks:

· Personal Care to include transfers, showering, toileting, and medication administration.

· Meal preparation and general cleaning

Ideal Candidate Experience Desired:

- All Personal Care

- Transfers

- Medication Administration

- Familiarity with Cerebral Palsy

- Pet Friendly

Usual Shifts In The Home: Monday – Sunday (we can discuss your ideal schedule)

· 8:30 am - 11:00 am

· 5:45 pm - 7:00 pm

· 10:15 pm - 11:45 pm

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
