Community Horizons is assisting in the recruitment process for a personal caregiver to provide excellent care to a fellow community member in her home. The member is seeking a caring, dependable, passionate caregiver to help maintain an independent lifestyle.
Schedule: Saturday and Sunday 8am-10am (every other weekend)
Wage: $15.00 per hour
Responsibilities:
· Assistance getting up and transferred utilizing a Hoyer Lift
· Personal Cares (Dressing & Bed Bath)
· Catheter Care
· Medication Support
· Light Housekeeping

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
