Description

The Case Manager will provide holistic case management services to CLA SoCal legal services clients. This full-time, non-exempt position reports to the Supervising Case Manager.
Responsibilities
  • Dependent on grant, provide services to a specific client population.
  • Dependent on grant, assist with grant reporting and tracking grant deliverables. Conduct bio-psycho-social assessments.
  • Provide array of case management services based on needs identified in assessment, present case findings to team and review case records.
  • Empower clients to become involved in their individual case planning and goal setting.
  • Maintain regular contact with assigned clients via client's preferred method of communication.
  • Serve as an emotional support for client during legal proceedings.
  • Observe confidentiality of attorney-client relationship.
  • Build rapport with clients, community partners and legal team to address concerns from a case management perspective.
  • Establish meaningful and effective relationships with community partners and conduct outreach events with community partners.
  • Identify community service needs or gaps and assists in the development of needed services and improvement in the quality of services.
  • Provide referrals to community resources and services and warm handoff to community partners for additional services.
  • Provide appropriate referrals for reassessment.
  • Create case management documentation including progress notes and case plans.
  • Support the creation of a client-centered culture that emphasizes client's autonomy and assists with restoring client's power and control.
  • Monitor progress of supportive services, as well as, evaluating availability and effectiveness of supportive services.
  • Provide case management perspective and trauma responsive education to legal team.
  • Collect data necessary to meet funding requirements and statistical reports.
  • Provide consultation and in-service training to staff.
  • Travel to meet client at a safe location for services, when appropriate.
  • Provide brief crisis intervention services for clients.
  • Perform other duties as required to meet the needs of the organization.

Requirements

  • Bachelor's degree required. Master's degree preferred. LCSW or related license desirable, but not required.
  • Two years' experience as a case manager or social worker - Preferred.
  • Articulate, creative, and passionate case manager with excellent written and oral communication skills, computer skills, and problem-solving skills.
  • Strong personal work ethic, initiative, and ability to organize time, and manage diverse activities with minimal supervision.
  • Strong interpersonal skill, client communication skills and comfort working with indigent clients.
  • Strong technical skills; including proficiency in Microsoft Office Suite (Word, Outlook, Excel, PowerPoint, Teams).
  • Desire to learn as well as share knowledge with other team members.
  • Empathetic and energetic attitude while working with others.
  • Ability to interact with a wide range of clients utilizing trauma-informed frameworks.
  • Valid driver's license, reliable vehicle, active insurance policy or other ability to travel frequently in and around the service area for representation and to engage with clients and community partners - Required.
  • Proven interest in social justice issues.
  • Ability to model CLA SoCal's values (compassion, collaboration, trust, accountability, and equity) in the performance of job duties - Required.
  • Commitment to CLA SoCal's mission - Required.
  • Bilingual proficiency - Preferred.
