Community Mainstreaming is a non-profit human service agency committed to providing services to help people with intellectual and developmental disabilities lead meaningful productive lives within their community.

We are seeking a Community Habilitation & Respite Coordinator to join our team! The Community Habilitation & Respite Coordinator is responsible for keeping records updated, complying with rules and regulations, and implementing policies and procedures directly related to staffing and day to day operations.

Locations: Nassau & Suffolk County, NY

Position : Full time
Hours : Monday – Friday, 9 am to 5 pm (40-hour work week)
Salary: $55,000

Responsibilities :

  • provides supervision to community habilitation and respite staff members who deliver community programs
  • handles day-to-day staff scheduling, addresses callouts, and manages staffing reassignments as needed
  • participates in new hire orientation for staff members and ensures ongoing training are completed
  • assists in identifying community resources and plans grouped recreational activities each month
  • facilitates grouped activities at the hub site
  • ensures seamless integration of services with all service providers
  • reviews service provision notations completed by community habilitation staff and participates in staff action plan development
  • reviews employee time data, approve staff mileage/reimbursement forms, and remain on call 24/7 for emergencies
  • conducts initial and annual evaluations of community habilitation and respite specialists
  • maintains ongoing contact with case management agencies (CMAs) and alternate service providers
  • available for on-call rotations as required for all waiver programs

Qualifications :

  • Bachelor’s degree preferred
  • Two to three years of experience working with individuals with intellectual and developmental disabilities
  • excellent communication skills
  • basic computer proficiency
  • valid New York State Driver’s license
