In this role, you will be responsible for providing psychosocial-emotional support and services to patients and families. At any given time, you will be working with 35-45 patients, meaning that you will always have the opportunity to learn and encounter a variety of patients. Individuals on this team make up half of the case management team, a group of 129 members. Each floor of our facility has 1-3 Social Workers on hand at all times, this means you will always be encountering a variety of specialties and types of care. As a member of our team, you will serve as a point of contact in order to assist patients and family in understanding, coping and adapting to the person, emotion and bio-psychosocial issues.
