SCHEDULE
This is a full time, hybrid position, 40 hours per week. Schedules will vary Monday through Friday and will require working some non-traditional hours to accommodate family schedules, however there is flexibility with supervisor approval and can be further discussed in the interview.
COMPENSATION & BENEFITS
Salary: $45,656 - $62,109 depending on experience
JOB SUMMARY
General Description: Responsible for coordinating a wide range of mental health services to families with children that have severe emotional disturbances. Responsible for developing, in a coordinated team approach, an individualized and comprehensive package of community services aiming at preserving family integrity and maintenance of the identified youth in a community setting. Responsible for facilitating assistance to children and families identified for the Wraparound process.
Responsibilities and Duties: Assesses, along with community partners and supervisor, the appropriateness of wraparound referrals through face-to-face interviews with referred families and completes a family-focused strengths-based assessment. Develops a family team with input from the family and the referring workers. Coordinates and conducts family team meetings aimed at developing and implementing a wraparound plan, a crisis plan, and a phase-out plan. Provides crisis intervention and ongoing coordination of services through frequent contacts with the family and all service providers. Advocates for families as necessary. Provides internal and external consultation and referral and treatment coordination as appropriate. Collaborates with other entities, including participation on the Child and Family Team and Community Team to ensure a maximum level of effectiveness. Provides consultation and coordination of services with other community agencies such as the Family or Probate Court. Adheres to the fidelity of the wraparound model and SEO waiver requirements. Collaborates with Department of Human Services on a consistent basis to facilitate identification of youth who would benefit from wraparound facilitation and/or SEO waiver application. May assist with public relation functions, when requested, which may include educational and training presentations. Completes all necessary documentation and data entry in the MSU evaluation project state database in a timely manner and prepares internal and external reports as needed. Stays current with professional credentialing and knowledge base within the scope of practice. Participates in staff meetings, committees and professional development activities.

EMPLOYMENT STANDARDS
General Education: Bachelor's Degree in Social Work, Psychology, Sociology, or other similar mental health specialty
Courses: Clinical areas, professional ethics
Training: Motivational Interviewing and Trauma Informed Care training preferred.
Job-Related Experience: Sufficient experience to perform the position at a satisfactory level required. Three years of experience with children and their families. CMHP required.
Licenses and Certifications: Michigan LBSW or individual with a human services degree hired and performing in the role of QMHP prior to January 1, 2008. Valid driver's license while operating vehicle on agency business.
CONDITIONS OF EMPLOYMENT
Knowledge: Knowledge of Person-Centered Planning; the characteristics and social aspects of mental and emotional disturbances; basic principles and techniques of psychosocial assessment, treatment planning, crisis intervention, mental health therapies, behavioral modification techniques, and community organization/intervention. Knowledge of legal systems, community agencies, and health, mental health, and social welfare programs. Knowledge of pertinent regulations, medications, and daily living skills as measured in CAFAS functional assessment tools.
Skills: Computer experience with windows environment to include clinical software. Verbal and written communication skills.
Abilities: Excellent ability to communicate both verbally and in writing. Ability to gather, record and evaluate information; monitor the needs of the recipient and their support system in order to formulate individualized treatment plans. Complete reports and reviews. Ability to work with individuals and families, and consult with other community agencies. Ability to lead teams and workgroups in interest of meeting family goals. Creativity and strong problem solving ability. Evening work required.
Physical Demands: Work is generally sedentary. Some travel is necessary.
Work Environment: Work is performed in a typical office setting, residences or other community settings and may involve dealing with people under very trying or difficult circumstances. Evening work required.
