We provide services to support independence and safety, helping those we serve to live their best lives.Role Of The Life Skills Assistant Program ManagerThe Life Skills Assistant Program Manager is responsible for assisting Individuals with intellectual and developmental disabilities (I/DD) achieve their highest level of independence. The Life Skills Assistant Program Manager focuses on providing care using a person-centered approach. It is the expectation that each Assistant Program Manager provides support and guidance in accordance with the scope of services to ensure the best outcomes towards outlined goals. An important role of the LSP Assistant Manager is to work in conjunction with the Program Manager to build a cohesive team. The LSP Assistant Manager inspires others, stimulates enthusiasm, and leads by example. The LSP Assistant Manager is committed to making a positive impact on others through effective relationship Essential Functions
Operations
Monitor the health and safety of all individuals
Assist with monitoring and implementing Individuals’ IP goals
Administer medication (medication certified required)
Attend Individuals’ support plan and IP program meetings, as needed
Provide administrative assistance to the Program Manager and Instructors
Coordinate and oversee custodial crew duties.
Work as an Instructor when needed
Communicate building maintenance issues and assist in coordination of resolution
Monitor daily classes to ensure are inclusive, engaging, and interactive
Familiarize yourself with behavior plans and
Identify Individuals with behavior plans, ensuring understanding of protocols, communicate protocols to appropriate instructors
Establish and maintain a supportive and professional relationship with individuals, their families, and circle of supports.
Provide individual-specific service in compliance with service descriptions as defined in service directory/specific attachments and core assurances
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the program
Recognize and note physical and/or emotional changes in an individual’s condition and seek appropriate intervention. Report all changes to program manager or director within 1 business day
Assist individuals with meals, personal hygiene, transitioning or other tasks, when needed
Assist with daily arrival/ dismissal duties and tasks.
Maintain supply inventory, communicate needs to the program manager to ensure operation of Individual IP goals and other program classes
Ensure documentation and submission of all incident reports in accordance with the APD guidelines
Communicate all incidents to the program manager
Assist with social media content
Coordinate community outings
Ensure building is cleaned and sanitized per CRN safety statement and standards
Administration
Complete all assigned documentation requirements. Including daily notes and summaries on progress towards support plan/ implementation plan goals
Contact program manager to report any difficulties, accidents, incidents or concerns immediately
Prepare General Event Reports using CRN documentation system and submit to required parties based on APD guidelines
Enter complete notes daily into CRN selected system.
Other Duties
Aid and assist individuals onboarding and offloading from transportation
Serve as a positive role model for adults in the program through appropriate dress, speech, attitude, and courtesy
Plan and implement activities that align with each Individuals’ program goals, enrichment, and physical activity utilizing curriculum and materials provided by the program
Comply with company policies and procedures
Maintain confidential information and ensure compliance with HIPAA
Attend all trainings as directed and provided by the employer as required of Med-Waiver providers
Other projects and duties as assigned
SUPERVISORY RESPONSIBILITY
No supervisory responsibility
Working Conditions And Physical Requirements
Primary activities are conducted within a well-lit, climate-controlled facility
Occasional community outings may require outdoor travel and potential exposure to unpleasant weather.
Ability to bend, stand, walk, or sit for extended periods of time and posses the normal range of body motion
Ability to regularly lift up to 50 pounds independently
Must be able to provide personal hygiene assistance to individuals requiring assistance
Periodic exposure to bodily fluids, and other pathogens
