Overview

JOB SUMMARY:

The Associate Director of Homeless Healthcare is responsible for the clinical oversight of clinical activities designated within homeless healthcare. This includes, but is not limited to, the provision of clinical care within shelter-based services, mobile services, street medicine, and permanent supportive housing. The Associate Director will serve as the organizational subject matter expert for care model development built around the lived experiences of people experiencing homelessness. The role will include working in tandem with fellow operational, nursing and clinical leaders within homeless healthcare and the broader intensive outpatient clinic service line. The Associate Director will collaborate with internal stakeholders and community partners to render and refine a comprehensive care continuum for marginalized populations that is reflective of local community needs. The Associate Director will collaborate with both internal partners and community stakeholders to analyze and assess the allocation of finite clinical resources relative to specific community needs. This individual will develop and routinely report out quality metrics that are reflective of strategic priorities within homeless healthcare and CommUnityCare. This position will also work closely with operational and marketing partners to direct and vet outreach initiatives for people along the homeless healthcare continuum. As an organizational leader the Associate Director of Homeless Healthcare may be expected to hold a clinical presence at multiple healthcare delivery sites along the homeless healthcare continuum. The Associate Director of Homeless Healthcare reports directly to the Director of Intensive Outpatient Clinics.

Responsibilities

ESSENTIAL FUNCTIONS: 

-Establish clinical standards of care for Homeless Healthcare services, inclusive of care pathways, protocols, and evidence-based and best practices, to establish and maintain the highest standards of quality, performance, and cost-effectiveness.-Organize and lead monthly homeless healthcare team meetings.-Support homeless healthcare clinicians through 1:1 mentorship-Work with the Director of Strategy to make sure IOC initiatives are in alignment with CUC’s strategic direction.-Work with the marketing team promote outreach and access of unique services to the community.-Serve as subject matter expert for the organization on optimal care models for people experiencing homelessness -Establish, update, and meet or exceed key quality and performance improvement goals in promotion of the mission and vision of the organization, inclusive of health equity, clinical outcomes, resourcefulness, workforce wellbeing, and community engagement.-Essential Functions-Develop strong community partnerships in support of the wellbeing of highly vulnerable populations.-Work with behavioral health and wellness partners to integrate trauma-informed practices into all elements of organizational structure.-In collaboration with clinical operations and nursing, promotes excellence in medical education among all learners.-Exemplify service delivery excellence-Serve as active participant of clinical leadership committees and meetings as delegated by the Director of Intensive Clinics or CMO

Primary Accountabilities:• Perform comprehensive assessment of patients using observation, inspection, auscultation, palpation and percussion.• Order or execute various tests, analyses and diagnostic images to provide information on patient’s condition.• Diagnose and treat diseases, disorders and injuries based on patient’s medical history and results of physical assessment and refer patients to medical specialist or other practitioner for specialized treatment.• Prescribe medications and therapies for treatment and preventative care.• Instruct patients and their families regarding procedures performed, home care and follow up visits.• Direct professional and ancillary health care staff during treatment of patients.• Respond to inquiries from staff and patients regarding preventative health and treatments.• Compile patient medical data including health history, treatment plans, and results of physical examination, etc. and record in electronic medical record.• Report required information including deaths and outbreak of contagious diseases to governmental authorities.• Plan, promote, demonstrate, recommend and coordinate high standards of medical practice.• Performs administrative duties as directed, up to 8 hours per week• Perform other duties as assigned.

Knowledge, Skills, and Abilities:

-High level of skill at building relationships and providing excellent customer services.-Interpersonal savvy and influence skills in managing difficult clients and patients.-High degree of knowledge and competency in the practice of medicine and associated charting requirements.-High level of problem solving skill to better serve patients and staff.-Strong attention to detail and accuracy.-Ability to utilize computers for data entry and information retrieval.-Ability to manage multiple responsibilities and emergency situations successfully.-Excellent verbal and written communication skills.-Ability to implement and evaluation operational and emergency situations successfully.-Demonstrated understand of and/or experience working in a patient centered medical home (PCMH) environment.-Demonstrated understanding and/or experience working in an integrated delivery environment.- 

Qualifications

Required Education:

M.D. or D.O. from an accredited educational institution.

Preferred Experience:

5 Years of patient care experience with 2 years in leadership/mentorship.

Required Licenses/Certifications (upon hire):

-unrestricted License to practice medicine in the state of Texas.

-Board Certification in Family Medicine or Internal Medicine or passage of Board Certification examination (within 1 year)

-Current Drug Enforcement Agency (DEA) registration for the purpose of writing prescriptions. 

-Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card from American Heart Association or American Red Cross.

-Basic Life Support Certification for Healthcare Providers.
