Duties & Responsibilities

• Complete assessment and intake services in a timely fashion for all assigned clients.
• Develop individualized service plans in conjunction with the family and other agencies/professionals.
• Document all services in accordance with Community Works policy and procedures and submit in a timely fashion. Ensure Medicaid and insurance guidelines compliance, as applicable.
• Plan and administer therapeutic treatment identified in the individualized service plan. Change method and degree of therapy when indicated by client reaction/needs.
• Collaborate with other professionals, family, and agency clinical staff to ensure consistency and timely completion of goals identified in the ISP.
• Maintain regular contact with other agencies/professionals and family regarding client progress via phone calls, letters, emails and/or monthly reporting. Prepare court reports as needed.
• Attend agency required meetings; i.e. staff meetings, treatment team meetings, IEP meetings.
• Comply with certification/licensing standards and provide all related documentation to administrative office.

Qualifications

MINIMUM REQUIREMENTS

Education:

• Master’s degree in human services field;
• Current license with the State of Oklahoma

Experience:

• At least 2 years experience in human services field

Physical:

• Must be able to lift & carry up to 25 pounds
• Full range of body motions
• Ability to spend 80%-90% of time standing, walking, kneeling and/or reaching to perform office duties;
• Sight, hearing, touch, & speaking is required 100% of time to perform office duties & interact with clients, consumers
