Description
*Collect and promote information pertaining to state and local individual and community preparedness activities/initiatives
* Identify and assist with mitigating barriers to program participation, including but not necessarily limited to, health, mental health, and housing.
*Care Coordinators in securing and identifying needed referrals to community and network medical, behavioral health and social assistance providers through telephonic, written, and/or face to face outreach, research, and maintenance of the program resource directory.
Qualifications
* High School Diploma/GED
* Microsoft Word and Excel
* Customer Service
* Good Telephone Etiquette
