**Job Description: Nurse Care Manager 1**
Responsibilities
- Support a caseload of approximately 225 clients with severe and persistent mental illness
- Provide behavioral and physical health support
- Offer training on medical diseases, treatments, and medications
- Identify gaps in care and assist in developing recovery plan goals for clients with chronic diseases
- Collaborate with CSSs and other team members on client health conditions
- Coordinate care and interventions as needed
- Contact medical providers and hospitals regarding admissions and discharges
- Conduct in-home visits and assist with medication administration
- Administer injections as required
- Maintain documentation of activities in client charts
- Champion healthy lifestyle changes
- Coordinate care with external healthcare providers
- Provide education on chronic disease states, medications, and healthy living
- Participate in team staffings and meetings
- Perform health screenings and identify wellness goals
- Record vitals and perform metabolic screenings
- Minimum of five (5) years of medical nursing experience preferred
- Experience in behavioral health and substance abuse prevention beneficial
- Current licensure as an LPN in the State of Missouri
- Ability to work in a multi-disciplinary team
- Excellent written and verbal communication skills
- Flexibility and ability to multi-task
- Occasional exposure to outdoor weather conditions
- Moderate noise level in the work environment
- Ability to lift up to 25 pounds
- Regular standing, walking, and sitting
- Visual acuity required for various tasks
