**Role and Responsibilities**
ResponsibilitiesAs part of our team, you will support a caseload of approximately 225 clients with severe and persistent mental illness, focusing on their behavioral and physical health needs. Your responsibilities will include:
This position requires the ability to frequently lift and move up to 10 pounds and occasionally lift and move up to 25 pounds. It also requires standing, walking, sitting, and manual dexterity for various tasks. Visual acuity is necessary for operating vehicles and performing tasks like data preparation and analysis.

**Role and Responsibilities**
ResponsibilitiesAs part of our team, you will support a caseload of approximately 225 clients with severe and persistent mental illness, focusing on their behavioral and physical health needs. Your responsibilities will include:
- Implementing a holistic, person-centered approach to care.
- Providing training on medical diseases, treatments, and medications to clients individually and in group settings.
- Identifying gaps in care and developing recovery plan goals for clients with co-occurring chronic diseases.
- Consulting with Integrated Health Specialists and team members regarding identified health conditions.
- Coordinating care with external healthcare providers.
- Focusing on healthy lifestyle changes including tobacco cessation, nutrition, physical activity, and substance use prevention.
- Conducting annual health screenings and identifying clients' health and wellness needs.
- Maintaining documentation in client charts and participating in team meetings for high-acuity clients.
- Administering injections and assisting in the clinic as needed.
This position requires the ability to frequently lift and move up to 10 pounds and occasionally lift and move up to 25 pounds. **Qualifications**
- Minimum of two years of medical nursing experience preferred, with a background in behavioral health and substance abuse prevention beneficial.
- Current licensure as an LPN or RN (Registered Nurse) in the State of Missouri is required.
- Strong ability to work within a multidisciplinary team, along with excellent written and verbal communication skills.
- Flexibility and the ability to multi-task are essential.
