Role OverviewAs a Nurse Care Manager, you will play a vital role in our person-centered integrated care team. Your responsibilities will include providing health interventions to individuals enrolled in our Health Care Home program. This position will involve working with clients in Raymore and Warrensburg, with occasional travel to Warrensburg.
Key Responsibilities

• Manage a case load of up to 200 Healthcare Home clients, ensuring completion of monthly interventions and care as per regulations.
• Conduct health screenings and assessments for assigned clients, identifying risks and making recommendations.
• Develop physical health goals and promote healthy lifestyles for clients.
• Facilitate referrals to primary care providers and specialists when necessary.
• Provide education to clients on various health topics and assist with medication management.
• Collaborate with healthcare providers and community agencies to ensure holistic care.
• Attend team meetings and participate in educational sessions aimed at improving healthcare services.

Requirements

• High School/GED required
• Practical Nursing Certificate or Associates/Bachelor's in Nursing
• LPN, RN, or GPN/GN with MO NCLEX exam required within 90 days of graduation
