Are you a nurse looking to be part of a dynamic healthcare team? Compass Health is seeking a dedicated Nurse Care Manager to join our primary care/healthcare home program. As a Nurse Care Manager, you will play a crucial role in our person-centered integrated care approach, providing direct clinical and medical services to individuals enrolled in our Health Care Home program.
Key ResponsibilitiesHealth Monitoring and Education: Meet with clients to conduct health screenings, provide monitoring, and offer education tailored to their medical needs.
Treatment Planning: Collaborate in developing treatment plans and healthcare goals for individuals with co-occurring chronic diseases.
Consultation and Education: Provide consultation and education to staff members on various health conditions and concerns.
Collaboration with Medical Providers: Communicate and collaborate with medical providers to ensure the best care for clients.
Data Analysis and Reporting: Review monthly care manager reports, analyze data, and communicate findings to relevant parties.
Coordination of Services: Assist in coordinating metabolic syndrome screenings, medication reconciliations, and other services as needed.
Cultural Competency: Seek culturally competent services and providers to meet the unique cultural needs of clients and their families.
Travel and Training: Be willing to travel to neighboring clinics for nursing coverage and training purposes.
Requirements
- High School diploma or GED required
- Associates or Bachelors degree in nursing required
- LPN or RN licensure required
