Clinical Liaison and Community Health Worker Role
As a Community Health Worker-Clinical, you will play a vital role in bridging the gap between health and social services within the community. Your primary focus will be to enhance access to services and elevate the clinical quality and cultural competency of service delivery. By engaging in activities such as clinical evaluation, outreach, community education, informal counseling, social support, and advocacy, you will empower individuals and communities to increase their health knowledge and self-sufficiency.
Key Responsibilities

• Act as a clinical liaison between health and social service communities
• Facilitate access to services and enhance service delivery quality and cultural competence
• Build individual and community capacity through various activities
• Provide clinical evaluations, outreach, community education, informal counseling, social support, and advocacy
• Improve community health by connecting health systems with community resources
• Offer clinical education on reducing behavioral health risk factors

Work ScheduleThis full-time position offers benefits and requires availability Monday through Friday from 8:00 am to 5:00 pm.
