Position OverviewThe Community Health Worker-Clinical at Compass Health Network plays a pivotal role as a clinical liaison between health and social service communities. This role focuses on enhancing service access, improving service delivery quality, and cultural competence. Responsibilities entail various activities such as clinical evaluation, outreach, community education, informal counseling, social support, and advocacy. The primary goal is to enhance Missourians' health by bridging health systems with community resources and providing clinical education on mitigating behavioral health risk factors.
Key Responsibilities
  • Facilitate patient appointments and arrange transportation resources
  • Collaborate with the Primary Care team to address self-management barriers
  • Provide culturally appropriate health information and education to patients
  • Encourage patients to take proactive steps towards health management
  • Enhance patient health literacy related to their conditions
  • Assist in securing clinical, social, and community resources for patients
  • Collaborate with stakeholders to support post-hospitalization care and patient goals
  • Document patient interactions and follow up on referrals and outcomes
  • Build relationships with community partnerships, healthcare, and social service providers
  • Participate in care coordination meetings and team collaborations
Requirements
  • Completion of a practical nursing program
  • High School diploma or GED
  • Licensed Practical Nurse (LPN) in Missouri
  • Successful completion of an accredited Community Health Worker program
