**Job Description**
Are you passionate about providing integrated and comprehensive care management services for adults with severe mental illness? Compass Health Network is currently seeking a Healthcare Home Nursing Director/Nurse Care Manager to join our team. In this role, you will be responsible for supervisory oversight, training, and coordination of Nurse Care Managers, as well as providing care to up to 100 Healthcare Home enrollees.
**Responsibilities**

• Develop and maintain relationships with primary and specialty care providers
• Coordinate hospital admissions and discharges
• Track enrollment, declines, and transfers
• Ensure compliance with all relevant standards and regulations
• Lead and manage a team of Nurse Care Managers
• Provide clinical supervision and oversight to staff
• Conduct regular team meetings and training sessions
• Collaborate with healthcare consultants and providers
• Travel to clinics and client homes as needed

**Qualifications**

• Registered Nurse degree from an accredited School of Nursing
• At least two years of experience working with the chronically mentally ill population
• Supervisory experience preferred

**Licensure/Certification**

• CPR, First Aid, and CPI certification required
• Licensed by the State Board of Nursing as a Registered Nurse
