Role OverviewAs a Nurse Care Manager at our organization, you will play a crucial role in our person-centered integrated care team. Your primary responsibility will be to provide direct clinical and medical services to individuals enrolled in our Health Care Home program, emphasizing a comprehensive approach to care.
Key Responsibilities
  • Manage a case load of up to 250 Healthcare Home patients
  • Conduct yearly care plan meetings with assigned clients
  • Assist in developing treatment plans and healthcare goals for individuals with co-occurring chronic diseases
  • Educate clients on medical needs and monitor their progress
  • Provide consultation and education to both community and office-based staff on identifiable health conditions
  • Collaborate with medical providers to ensure comprehensive care for patients
  • Deliver education to clients on medical diseases, treatment options, and medication management
  • Monitor HIT tools and reports for treatment and medication alerts, as well as hospital admissions/discharges
  • Review monthly DRVS reports, analyze data, and communicate effectively with providers, clinicians, and patients
  • Conduct Medication Reconciliations for HCH enrollees post-hospital discharge
  • Assist in UDS measure documentation, tracking, and reporting
  • Work collaboratively as part of a multi-disciplinary team to ensure holistic care
  • Seek culturally competent services and providers to meet the diverse cultural needs of our clients
EducationEssential Qualifications:
  • High School diploma/GED required
  • Practical Nursing or Associate of Nursing degree required
  • Bachelor's degree in nursing preferred
Experience
  • Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
Licensure/Certification
  • Registered Nurse (RN) required
