Role OverviewAs a Nurse Care Manager at our organization, you will play a crucial role in our person-centered integrated care team. Your primary responsibility will be to provide direct clinical and medical services to individuals enrolled in our Health Care Home program, emphasizing a comprehensive approach to care.
Key Responsibilities
- Manage a case load of up to 250 Healthcare Home patients
- Conduct yearly care plan meetings with assigned clients
- Assist in developing treatment plans and healthcare goals for individuals with co-occurring chronic diseases
- Educate clients on medical needs and monitor their progress
- Provide consultation and education to both community and office-based staff on identifiable health conditions
- Collaborate with medical providers to ensure comprehensive care for patients
- Deliver education to clients on medical diseases, treatment options, and medication management
- Monitor HIT tools and reports for treatment and medication alerts, as well as hospital admissions/discharges
- Review monthly DRVS reports, analyze data, and communicate effectively with providers, clinicians, and patients
- Conduct Medication Reconciliations for HCH enrollees post-hospital discharge
- Assist in UDS measure documentation, tracking, and reporting
- Work collaboratively as part of a multi-disciplinary team to ensure holistic care
- Seek culturally competent services and providers to meet the diverse cultural needs of our clients
- High School diploma/GED required
- Practical Nursing or Associate of Nursing degree required
- Bachelor's degree in nursing preferred
- Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
- Registered Nurse (RN) required
