Exciting Opportunity at Compass Health - Nurse Care Manager Position
Compass Health is seeking a dedicated nurse to join our dynamic team as a Nurse Care Manager in our primary care/healthcare home program. As part of a person-centered integrated care team, the Nurse Care Manager will provide direct clinical and medical services to individuals enrolled in our Health Care Home program, using a comprehensive whole person approach.
Key ResponsibilitiesHealth Screening and Monitoring: Conduct health screenings, educate clients based on their medical needs, and monitor their progress.
Treatment Planning: Assist in developing treatment plans and healthcare goals for individuals with co-occurring chronic diseases.
Consultation and Education: Provide consultation and education to community and office-based staff on various health conditions.
Collaboration with Medical Providers: Collaborate with medical providers on significant health information and ensure streamlined communication.
Data Monitoring and Analysis: Monitor HIT tools and reports for alerts related to treatment, medication, hospital admissions, and discharges. Analyze monthly care manager reports and communicate effectively with stakeholders.
Specialized Duties: Coordinate Metabolic Syndrome Screenings, conduct Medication Reconciliations for discharged HCH enrollees, and interface with other human/social service agencies to provide holistic services.
Cultural Competency: Seek culturally competent services and service providers to meet the unique cultural needs of clients and their families.
Training and Coverage: Travel to neighboring clinics for nursing coverage and training as required.
QualificationsHigh School diploma/GED required
Associates or Bachelors in nursing required
LPN or RN license required
