As a Nurse Care Manager, you will play a crucial role in integrating physical health care into the behavioral health setting, ensuring individual and population-based health interventions for our Health Care Home clients.
Duties and Locations:

• Work in Columbia office on Monday & Wednesday, Jefferson City Metro office on Tuesday, Thursday, & Friday weekly
• Responsible for managing up to 200 Healthcare Home clients
• Conduct health screenings, develop wellness plans, and promote healthy lifestyles
• Ensure clients have a primary care provider, complete metabolic screenings, and provide medication reconciliations
• Educate clients on physical health conditions and facilitate referrals to relevant healthcare providers
• Collaborate with multi-disciplinary teams and provide culturally competent care

Requirements

• High School/GED and LPN or RN certification
• Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
