Job DescriptionAs a Nurse Care Manager at Compass Health Network, you will be an integral part of our person-centered integrated care team. Your role involves providing individual and population-based health interventions to persons enrolled in the Health Care Home, by integrating care for physical health conditions into the behavioral health setting. This position requires daily travel between Jefferson City and Columbia locations.
Responsibilities
- Complete monthly assigned interventions for up to 200 Healthcare Home clients
- Conduct health screenings and identify risk levels for clients
- Develop physical health goals and promote healthy lifestyles
- Ensure clients are referred to Primary Care Physicians or Specialists
- Perform Metabolic Syndrome Screenings and Medication Reconciliations
- Provide education on physical health conditions and self-management
- Collaborate with medical providers and social service agencies
- Attend team meetings and agency meetings as required
- Travel to clinics, client homes, and other locations as needed
- Adhere to company guidelines and service provision expectations
- High School/GED required
- Practical Nursing certificate or degree in nursing required
- Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) required
