Role OverviewAs a Nurse Care Manager, you will play a crucial role in our integrated care team, focusing on individual and population-based health interventions for Health Care Home clients in Raymore and Warrensburg.
Key Responsibilities
- Manage a case load of up to 200 Healthcare Home clients, ensuring completion of monthly interventions and care according to regulations
- Conduct health screenings, assess risk levels, and develop physical health goals
- Educate clients on the importance of regular healthcare provider visits and promote healthy lifestyles
- Collaborate with medical providers and social service agencies to provide holistic care
- Attend team meetings and provide consultation and education for staff on health conditions
- High School/GED required
- Practical Nursing Certificate or Associates/Bachelor's in Nursing
- LPN, RN, or GPN/GN licensure, with MO NCLEX exam completion within 90 days of graduation
