Join our dynamic team at Compass Health Network as a Hospital Access Liaison (HAL)!
Responsibilities**Client Care**

Responsibilities**Client Care**
- Provide immediate assistance and support to individuals transitioning from the Emergency Department or inpatient unit
- Utilize short-term, community-based interventions for up to 90 days post initial contact
- Available for travel to meet clients in the community
- Maintain regular communication with hospital staff to ensure seamless care and address any concerns
- Conduct follow-up calls and outreaches to referred individuals
- Attend regular meetings and clinical supervision sessions
- Uphold productivity standards
- Deliver culturally competent services tailored to individual needs
- Stay informed about all services offered by Compass Health
- Keep up-to-date on other social services agencies for potential referrals
- Maintain accurate client records in compliance with agency and state regulations
- Ensure all documentation is concise and professional
- Work in conjunction with other programs to facilitate referrals when necessary
- Collaborate with multidisciplinary treatment teams when clients are referred
- Master's degree in social work, psychology, or related field
- Minimum of three years of experience in behavioral health
- Familiarity with DSM-V
- Valid MO driver's license with minimum automobile coverage as established by the agency
- Preferred: PLPC, LPC, LMSW, LCSW, PLP, or LP
