Are you a dedicated Nurse LPN looking to be part of a person-centered integrated care team? Compass Health Network is seeking individuals who are passionate about providing specialized services to primary health care patients. As a Nurse LPN (Outpatient) with us, you will have the opportunity to assist in managing the full array of physical and behavioral health care needs of our clients through a holistic approach.
**Schedule and Locations:**

• Work hours are Monday to Friday.
• Shift times vary: some start at 07:00 and end at 17:30, while others end at 13:00.
• Wednesday clinics will be split between Osceola and Clinton.

Essential Functions - Job Specific

• Prepare patients for examination and treatment.
• Take patient histories, vital signs, and document in patient charts.
• Set up exam and treatment rooms with necessary instruments and supplies.
• Perform venipunctures, urinalysis, EKGs, and other procedures as directed.
• Administer immunizations and injections.
• Phone in patient prescriptions and maintain documentation.
• Provide patient education and instructions.
• Prepare and maintain supplies and equipment, including sterilization.
• Assist in minor surgeries and physicals.
• Help schedule tests and treatments.
• Screen and handle telephone calls and messages appropriately.
• Maintain patient files, records, and information.
• Adhere to standard precautions, CLIA, and OSHA regulations.
• Assist in the cleanliness and maintenance of unit facilities and equipment.
• Monitor and order medical supplies and equipment.
• Collaborate with medical providers.
• Educate patients on diseases, treatments, medications, and self-management tools.
• Monitor HIT tools and reports for alerts, admissions, and discharges.
• Triage medical questions and requests for refills.
• Serve as a medical/nursing consultant to staff and patients.
• Follow up with discharged patients for review and planning.
• Provide culturally competent services and support.

Education

• High School/GED required.
• Associate degree in nursing preferred.

Work Experience

• At least two (2) years of experience as a licensed practical nurse preferred.
• Proficiency in healthcare technology software preferred.

Licensure/Certification

• Licensed Practical Nurse required.
