Role DescriptionAs a Nurse LPN/RN in the Outpatient setting, you will be an integral part of a person-centered integrated care team. You will deliver specialized services to primary health care patients, managing their physical and behavioral health care needs through a comprehensive approach.
Key Responsibilities
- Prepare patients for examinations and treatments
- Take patient histories, vital signs, and document in charts
- Administer medications and injections
- Perform procedures such as venipunctures, urinalysis, and EKGs
- Assist in minor surgeries and physicals
- Educate patients and provide instructions
- Maintain supplies, equipment, and sterilization processes
- Coordinate tests and treatments
- Screen and triage telephone calls
- Adhere to regulations and standards for safety and cleanliness
- Monitor medical stock and supplies
- Provide patient education on medical conditions and self-management
- Act as a consultant to clinical staff
- Ensure culturally competent services are provided
- High School diploma/GED required
- Associate degree in nursing preferred
- Minimum of two years of experience in a primary health care setting
- Proficiency with healthcare technology software is preferred
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) required
