**Job Overview**
The Nurse LPN Outpatient plays a crucial role in delivering specialized services to primary health care patients in an office-based setting. This position focuses on managing the full spectrum of patients' physical health care needs, as well as their behavioral health care needs, utilizing a holistic "whole-person" approach.
**Schedule**
This position requires a commitment of 40 hours per week, primarily Monday to Friday. Additionally, there is one mandatory Thursday shift per month from 9:00 am to 6:00 pm.
**Essential Job Functions**

• Prepare patients for examinations and treatments, including documenting patient information
• Set up exam and treatment rooms with necessary equipment
• Conduct procedures such as venipunctures, urinalysis, EKGs, and administer immunizations and injections
• Handle phone communication for patient prescriptions and maintain accurate documentation
• Provide patient education and instructions
• Maintain and organize medical supplies, ensure sterilization of equipment
• Assist in preparations for minor surgeries and physicals
• Coordinate appointments for tests and treatments
• Screen and manage telephone calls, messages, and documents
• Manage patients' records and information in compliance with regulations
• Adhere to safety protocols and regulations
• Assist in maintaining cleanliness and organization of unit facilities and equipment
• Monitor and manage medical stock and laboratory needs, ordering supplies when necessary

**Additional Responsibilities**

• Collaborate with medical providers and facilitate communication regarding patient care
• Educate patients on medical conditions, treatments, medications, and self-management practices
• Monitor HIT tools and reports for relevant alerts and information
• Triage medical inquiries and refill requests according to established protocols
• Provide medical/nursing consultation to staff and patients
• Support discharged patients with follow-up plans and appointment scheduling
• Champion culturally competent services for all agency consumers and families

**Other**

• Perform other duties as determined by the supervisor to support clinic operations
