The Nurse (RN) for the Assertive Community Treatment for Transitional Aged Youth (ACT TAY) Team is responsible for:
  • Conducting psychiatric assessments.
  • Assessing physical health needs.
  • Making appropriate referrals to community physicians.
  • Managing and administering medication in conjunction with the psychiatrist.
  • Providing a range of treatment, rehabilitation, and support services for individuals in Assertive Community Treatment services.
Assertive Community Treatment (ACT) is a method of delivering many types of service to people with chronic mental health disorders. ACT teams provide services directly to individuals in their community, ensuring that they receive wrap-around psychiatric services within the comfort of their own homes. Team members are trained in various fields including psychiatry, social work, nursing, substance use disorders, and supported employment, which allows them to meet the diverse needs of those they serve. The ACT team operates 24 hours a day, seven days a week, throughout the entire year.
