Senior Director of Nurse Care Manager Services in Any Compass Health Network location, Missouri at Compass Health Network The Sr. Director of Nurse Care Manager (NCM) Services is a member of the Healthcare Home (HCH) Leadership Team and directs and manages a team of Regional Nurse Directors (RND) as well as the Nurse Floater/Trainer, providing guidance and supervision on all aspects of the HCH Nurse Care Management Services. Position location flexible to existing office locations; open to Missouri residents. ESSENTIAL FUNCTIONS – JOB SPECIFIC:
  • Champion integration of physical and behavioral health, prevention, and wellness through quality assurance and improvement
  • Work collaboratively with CPRC/SUD/Outpatient Nursing and other clinical leadership to ensure that health home activities are integrated into other clinical programs as appropriate: o Maintain communication with other department directors and supervisors regarding program/client needs o Provide feedback to VP Healthcare Home regarding issues, developments, and needs regarding NCM services o Work with HCH Leadership Team on analysis of data and actions to be taken to ensure Continuous Quality Improvement o Work with Specialized Healthcare Consultants and RNDs to arrange training on identified health topics for staff
  • Lead and manage a team of Regional Nurse Directors and the Nurse Floater/Trainer: o Provide and document monthly supervision to all direct reports incorporating feedback on data from outcomes and other reports; conduct AIM discussions o Set the agenda for and lead regular team meetings; bringing forward issues as needed to the HCH Leadership Team; disseminate information regarding policies/procedures and Compass information from other meetings (i.e. Regional VP meetings, HCH Leadership, Management Team, etc) o Ensure adequate staffing patterns to meet needs and onboard new direct reports as needed o With RNDs, ensure optimal enrollment to slot capacity for each team o With RNDs, monitor dashboards and other reports to ensure completion of all required nursing interventions; assist in arranging for assistance in completion when needed o Ensure proper documentation of all HCH nursing services by conducting quality chart monitoring on a representative number of charts per month; provide feedback to the RNDs and the leadership team as needed o Prioritize where the services of the Nurse Floater/Trainer are needed o Work with the Nurse Floater/Trainer and QM Director to ensure that NCM processes are kept up to date o Provide backup to any direct report position as needed
  • Attend Regional CPRC/SUD huddles and integration meetings as able, HCH Leadership meetings, Compass Management Team meetings, Regional VP meetings, Nursing Integration meetings, Agency and State level HCH meetings and trainings, and other meetings as assigned
  • Other duties as assigned SUPERVISORY RESPONSIBILITY:
  • Oversees day-to-day operations and tasks of supervisees
  • Completes performance development/AIM process
  • Time card management – manages time sheets
  • Ensures progress and development is made toward departmental strategic plan goals Job Requirements
  • Associates/ bachelor’s degree in nursing required or
  • Bachelor’s degree in the human services field (social work, psychology, counseling, etc.) and relevant experience required At least two (2) years of supervisory experience preferred; Experience in the Intellectual Developmental Disability (IDD) field required.
