Are you passionate about providing quality and compassionate care to communities in need? We are seeking a Nurse (LPN/RN) to join our Assertive Community Treatment for Transitional Aged Youth (ACT TAY) Team. In this role, you will be responsible for conducting psychiatric assessments, addressing physical health needs, referring clients to community physicians, and managing medication administration, among other essential duties.
What is Assertive Community Treatment (ACT)?
ACT is a comprehensive approach to supporting individuals with chronic mental health disorders in their own communities. Our ACT team, comprised of professionals trained in psychiatry, social work, nursing, substance use disorders, and supported employment, provides services 24/7, every day of the year.
Responsibilities

• Collaborate with clients and the treatment team to develop, implement, and revise treatment goals and plans
• Provide individual therapy and symptom management
• Assist in medication administration and integration into treatment plans
• Educate clients and families on medications and mental health
• Conduct psychiatric assessments and maintain documentation
• Participate in crisis intervention and client support

Requirements

• Associate's degree in nursing
• Experience in psychiatric settings
• LPN or RN licensure in Missouri
