Compass Healthcare Consulting & Placement is searching for an experienced Maternal Child Health RN for Home visits for a position with a large hospital system in the Bronx, NY. Qualified candidates will have a valid NYS RN as well as prior Maternal Child Health RN experience within a clinical setting such as hospital, Labor & Delivery, NICU or Postpartum units. Great opportunity for an experienced RN to work with a great program! Visit patients, mothers and babies, to continue their care at home.

Position Responsibilities:

• Assesses, plans, implements,and evaluates the patient’s plan of care

• Provides direct skilled nursing care and observes and instructs the patient in their homes

• May coordinate services of other caregivers

• Provides individualized skilled Nursing care to the patients

• Develops a plan of care with ongoing evaluation of patient/family needs for other services

• Complete thorough and accurate nursing assessments for the Maternal Child Health (MCH) patients

• Reviews and revises plan of care as necessary, consistent with physician's orders

• Authorize & coordinate all services including nursing, rehab, respiratory therapy, nutritional services, equipment, professional and non-professional staffing and support services

• Supervises and monitors the care given by home health aides and ensures proper documentation

• Provides mother/pediatric and/or caregiver with teaching and involves them in assessment activities. Provides teaching on disease and medication management

• Ability to apply evidence-based practices to the home care setting

• All other duties as requested

Qualifications:
