Compass Healthcare Consulting & Placement is conducting a search for a Nurse Practitioner, FNP ANP or GNP. Opportunities available in Manhattan/Bronx region. Qualified candidates will be a Board Certified Nurse Practitioner in the state of NY, will have some working experience in Primary Care, Internal Medicine and/or Long-Term Care and will have compassion, dedication and desire to provide quality care to plan's members within the community. Working with a Managed Care Insurance Plan, NP is responsible for creating plans of care and facilitating care to Members who live within the community setting.

Nurse Practitioner - Manhattan/Bronx, NY

In collaboration with physicians, the NP will be providing comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self - management education & tools to better manage patient’s multiple conditions. As a part of the Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to assigned at-risk patients who reside in the community. The NP will ensure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as a clinical resource to the member, family and interdisciplinary team.

Responsibilities:

  • Provide and/or coordinate primary clinical care services for assigned high risk members and facilitate supportive care during transitions from acute or sub-acute care settings.

  • Performs Comprehensive Clinical Assessments in the home setting, or in the facility, and develops plan of care utilizing evidence based practice recommendations

  • Collaborates with Plan Case Manager to schedule and follow-up home visits to update the care plan.

  • Ensures safe transition from setting to setting, conducts post discharge assessment within a reasonable time period

  • Evaluates appropriateness of member transfers for acute inpatient services, prospectively and retrospectively and shares necessary clinical information with collaborating physicians/facilities

  • Orders diagnostic services and/or therapeutic procedures as necessary to maintain member health

  • Provides, or ensures, that annual preventive care services are administered and properly documented in the medical record

  • Works in continuous collaboration with primary care physician

  • NP will provide individualized care for the at-risk population that would improve care coordination, reduce avoidable utilization of hospitals, improve the health and quality of life of community dwelling

  • Acts as a team player, member’s advocate, and a clinical lead while working closely with Interdisciplinary Team to coordinate member’s care and optimize health outcomes

  • Acts as resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

Qualifications:

  • Master’s Degree in Nursing; graduate of accredited Nurse Practitioner program required.

  • One year of clinical experience in role of nurse practitioner required; Minimum one year in geriatric specialty

  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred

  • Prior experience in managed care a plus

  • Ability to assess, diagnose, treat and follow-up

  • Ability to work independently and as part of interdisciplinary teams

  • Ability to develop relationships and communicate to members/family

  • Establish priorities and maintain schedules

  • NYS RN/NP License, Registration, DEA certificates

Excellent Salary $138,000 - 150,000 and Benefits Package including $15k Sign On Bonus!
