Compass Healthcare Consulting & Placement is conducting a search for an experienced Speech Language Pathologist, SLP, for a opening in a Certified Home Healthcare Agency (CHHA). Qualified candidates will have prior Home Health experience as a Speech Therapist, will have ability and knowledge to provide field visits to patients in the community and provide education to patients and families. SPEECH LANGUAGE PATHOLOGIST - SLP 

OBJECTIVE

• To make visits to patients in the community

• To ensure therapy services are provided according to agency’s Policies and Procedure in compliance with all State, Federal and local laws and regulations.

• Provide quality speech therapy intervention as ordered by the Primary Physician to a specific caseload.

REQUIREMENTS

Provide skilled therapy interventions in the treatment of the patient/client diagnosis, rehabilitative needs and preventative care.

1. Provides ongoing assessments, problem identification and specific therapeutic intervention, including teaching and training activities.

2. Fulfills job description for Speech Language Pathologist

3. Perform all indicated performance and competencies as indicated per agency’s Policies & Procedures.

4. Ensure compliance with all paperwork, including Home Health Aide supervision, OASIS, 60 Day Summaries, Discharge Summaries etc.

5. Participates in PAC and QI and other committees as required.

6. Other duties as needed.

7. Performs initial assessment visits on Speech Therapy only clients.

8. Utilize a systematic, individualized, goal/outcome driven approach in developing the therapy plan of care in the delivery of therapy services. Ensures proactive revision or addition to the plan of care to meet the patient/client needs as assessed or diagnosed, or as required by policy/regulation.

9. Assess the appropriateness of paraprofessional care related to therapy, and appropriately incorporates problem specific interventions in the paraprofessional plan of care, as it relates to speech therapy.

10. Confer with the patient/client’s physician in developing, implementing and revising the therapy Plan of Treatment, based on initial and ongoing assessments, and as required by policy/regulation. Adheres to the Plan of Treatment in the provision of care.

11. Ensure any changes in treatment plan or frequency are communicated to the DON/designee so that the corresponding interim orders can be obtained.

12. Initiate and respond appropriately to contact with/from patient/client, physician, Director of Nursing or designee, other members of the healthcare team, as per agency’s policy, as it regards patient/client family needs and status changes.

13. Understand the emergency/disaster preparedness plan and procedures in place to meet the patient/clients individualized needs.

Qualifications

Academic

The candidate for this position must be a Speech Therapist (Pathologist) who is licensed as required by Article 159 of the New York State Law. The candidate must have at least obtained a Master’s Degree in Speech Language Pathology approved by the New York State Education Department. This candidate requires acute or home care experience within the last two years. All accepted candidates must have a minimum of two years of clinical experience in their area of specialty.
