Compassionate Care Services is seeking a passionate and self-motivated self-starter to provide support coordination services to adults, age 21 and over, with developmental disabilities.
Responsibilities
1. Work closely with individuals and families to learn about their skills, talents and interests, and then link them to appropriate supports and services in the surrounding community.
2. Create an individualized service plan and monitor and adjust as needed. The
3. Assist in managing the budget that is allocated to them by DDD for various services and supports.
4. Attend monthly webinar trainings that are held by DDD for CEU requirements.
5. Other tasks as needed
Qualifications
1. Bachelor's degree
2. At least 2 year documented professional experience working with individuals with disabilities.
3. Prior support coordination experience a plus.
4. Working knowledge of programs and services, benefits, entitlements and supports available to persons with disabilities, as needed.
5. Independent thinker and self starter
6. Time management skills a must
7. Spanish speaking a plus
