Respirator fit testing (quantitative)
Respirator medical evaluation questionnaire review
Health history review
Review of any significant changes that may need further testing
Blood pressure
Pulse
Audiogram
Review of cardiac system
Review of pulmonary system
Pulmonary function test (if required by NP/MD)
Perform Additional Screening/testing Every 3 YearsPulmonary function test
Drug screen (urinalysis) / breathalyzer
Vision testing (VA far, peripheral, and color vision)
EKG (resting)
Blood Chemistry Profile To IncludeCBC with differential
Creatinine or BUN
Glucose
Cholesterol
Triglycerides
HDL
Total bilirubin
SGOT
SGPT
Prepare results of all the above testing for review by NP/MD
Document all testing results in accordance with established guidelines (hard copies for employee files and data entry/charting in Cority)
Ensure compliance with regulatory and corporate guidelines
Communicate with employees, Operations Lead and Maintenance Lead, and health care providers on health and fitness-for-duty issues
Informs Operations Lead and Maintenance Lead of upcoming testing due dates and situations in which employees require additional off-site testing/clearance/approval (information to be provided in accordance with HIPAA guidelines)
Performs work site visits and maintains current knowledge of local hazards, toxicity of materials, and relevant emergency medical care
Additional Ad-hoc NeedsAssists in preparing patients for off-site transport during medical emergencies. Provides nursing treatment of work-related injury and illness. Assists in administration of case management for prolonged or complex illnesses/injuries.
The following equipment will be available for use to complete on-site testing: Audiometer, EKG, breathalyzer, spirometer (pulmonary function), titmus vision screening, AED, pulse oximeter, blood centrifuge, exam room, Quantifit Respirator Fit Tester
