Responsibilities Include:
- Assist with coordination and implementation of clinical activities that provide culturally competent service planning to help residents with identifying needs according to the mission of the program.
- Provide assistance to mental health counselors with emphasizing psycho-education, medication management, money management, entitlements, securing permanent housing and activities of daily living.
- Maintain resident-based caseload as determined by the needs of the program.
- Complete weekly progress notes in accordance with Medicaid billing procedures.
- Coordinate crisis intervention with mental health and/or substance abuse providers such as hospitals, detoxification clinics, day treatment programs, therapeutic communities, etc.
- Report to Program Director all deficiencies within the facility to assure all safety standards are met in accordance with program policy and mandates.
REQUIRED KNOWLEDGE and SKILLS
- Working knowledge of Principles and practices of Comunilife Human Resources policies and procedures and executive directives.
- Working knowledge of regulations for Part 595 and Medicaid funding and reimbursement procedures.
- Responsible, organized, effective, and efficient.
- Ability to perform within a team and accomplish multiple assignments simultaneously.
- Effective written and oral communication skills, ability to employ active listening skills, and computer literacy.
- Knowledge of New York State Office of Mental Health and New York City Department of Health and Mental Hygiene system (DOHMH), Housing and Urban Development (HUD), providers and related resources.
REQUIRED EDUCATION and EXPERIENCE:
- Associates degree in Health/Human Service or (BA/BSW) preferred; may be substituted by four years of mental health counseling and/or administrative experience in mental health/housing experience with supervision by a licensed professional.
- Training in conflict resolution and solid clinical skills
- Experience working with mentally ill individuals

In 2017, Comunilife met the needs of more than 2,100 low-income and vulnerable New Yorkers. Roughly 90% of our clients have incomes below the federal poverty line; 90% are African-American or Latino; and 50% have Spanish as their primary (or sole) language. All struggle with profo...und challenges, i.e. chronic homelessness; HIV/AIDS and other major medical issues; serious mental illnesses and substance abuse. Comunilifes affordable and supportive housing programs address the need for safe, affordable, service enriched housing for a low-income population that has difficulty maintaining stable housing. A majority of our housing clients have a history of homelessness and/or long-term hospitalizations. Many are living with mental illness, HIV/AIDS and other chronic illnesses. Comunilife has 1806 apartments/units in 10 supportive residences, 3 low-income apartment buildings and 981 scatter site apartments in the Bronx, Brooklyn, Queens and Manhattan. Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate. Comunilife is a leader in the field of Latina adolescent suicide prevention. Our Life is Precious Program, which is celebrating its 10th anniversary in 2018, provides non-clinical services to girls ages 12 to 17 in the Bronx (2008), Brooklyn (2009) and Queens (2015).Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
