Health Center is now seeking a full time LCSW to provide triage, warm hand-offs and counseling interventions for patients who receive primary care at the Health Center as part of an integrated Care Team. The therapist is part of the team providing whole person care, which may include assisting with Medication Assisted Therapy, referring to partner agencies, case management and crisis response within clinic hours. This clinic uses the Collaborative Care Model from University of Washington, AIMS center. Work will be in clinic. Great team atmosphere.

Experience

• Mental Health, 1 year
• Substance Abuse, 1 year
• Must enjoy working with at risk populations.
• Bilingual/Spanish preferred, but not required.

Education

• Master's degree in Social Work from an accredited college.
