About the Role

The Company is seeking a Vice President of Population Health to lead and advance its population health initiatives. The successful candidate will be responsible for strategic leadership in improving health outcomes, managing healthcare costs, and enhancing the quality of care for the patient population. This role requires a strong background in healthcare management, with a minimum of 10 years of experience, and at least 5 years in a leadership position focused on population health or care management. The VP of Population Health will be at the forefront of developing and implementing strategic plans, and must have a Master's degree in a relevant field, and preferably hold ANCC-recognized Care Management certifications.Applicants for the VP of Population Health position at the company should possess a Master's degree in Public Health, Healthcare Administration, Nursing, or a related field, and demonstrate a deep understanding of population health principles, care coordination, and quality improvement methodologies. The role demands excellent analytical, communication, and interpersonal skills, as well as experience with data analytics and health information technology. The ideal candidate will have a proven track record in program development, implementation, and process improvement, and the ability to work collaboratively across disciplines. Strong leadership and teaching skills are essential for fostering a cohesive and successful team, and the candidate should be adept at meeting established targets and deadlines with strong organizational skills.
