Job Title: Primary Care Nurse Practitioner
Locations: Plainview, TX / Giddings, TX / Granbury, TX/ Mineral Wells, TX

JOB DESCRIPTION

This position offers a rewarding opportunity for a Primary Care Nurse Practitioner to provide high-quality care to residents across multiple locations in Texas, including Plainview, Giddings, Granbury, and Mineral Wells. The role emphasizes comprehensive patient care, requiring a commitment to diagnosing and managing a variety of health conditions, particularly in long-term care settings.

RESPONSIBILITIES

  • Provide comprehensive healthcare services and coordinate patient care
  • Work closely with a team of healthcare professionals
  • Assess, diagnose, and treat residents' physical and mental well-being
  • Conduct thorough patient assessments, including medical history reviews and physical examinations
  • Order and interpret diagnostic tests
  • Develop accurate diagnoses and treatment plans
  • Prescribe medications and administer treatments
  • Provide counseling to residents and their families
  • Collaborate with the interdisciplinary team for seamless coordination of care
  • Manage chronic conditions and monitor progress
  • Make appropriate adjustments to care plans
  • Commitment to person-centered care
  • Create a safe and nurturing environment for elderly residents in the nursing home

REQUIREMENTS

  • Active Nurse Practitioner License in the state of Texas
  • Active DEA License
  • Minimum 6 months experience in long-term care
  • Experience in primary care or family medicine preferred
  • Strong diagnostic skills and clinical judgment
  • Excellent communication and interpersonal skills
  • Proficiency in electronic health record (EHR) systems
