Job Description:
Leadership: Provide strategic leadership and oversight of the care management department, ensuring high-quality patient care and efficient operations.
Staff Supervision: Manage and support care management staff, including hiring, training, and performance evaluations.
Program Development: Develop and implement care management programs and initiatives that enhance patient outcomes and reduce hospital readmissions.
Compliance: Ensure compliance with all federal, state, and local regulations, as well as accreditation standards.
Interdisciplinary Collaboration: Work collaboratively with medical, nursing, and administrative staff to coordinate patient care and streamline processes.
Quality Improvement: Lead quality improvement efforts within the department, utilizing data-driven approaches to enhance care delivery.
Resource Management: Oversee budget management, resource allocation, and financial performance of the care management department.
Patient Advocacy: Advocate for patient needs and ensure that care plans are patient-centered and evidence-based.
Education Certification Requirements:
Bachelor's degree in Nursing (BSN) or a related field; Master's degree preferred.
Current RN licensure in the state of Missouri.
Minimum of 5 years of experience in care management, with at least 2 years in a leadership role.
Certification in Case Management (CCM) or equivalent preferred.
Strong knowledge of care management principles, practices, and regulations.
Excellent leadership, communication, and organizational skills.
