Act as a liaison between patients, families, and healthcare providers to ensure effective communication and continuity of care.
Coordinate patient admissions, transfers, and discharges.
Assess patient needs and develop personalized care plans.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Provide education and support to patients and their families regarding treatment plans and care procedures.
Maintain accurate and up-to-date patient records.
Ensure compliance with healthcare regulations and standards.
Education And Certification Requirements:
Current and unrestricted RN license in the state of California.
BSN preferred, but ASN with relevant experience will be considered.
BLS and ACLS certifications required.
Previous experience in a clinical liaison role or similar position is highly desirable.
Strong communication and organizational skills.
