Job DescriptionJoin Our Team as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) at a Premier SNF/Rehab Facility near Ontario, OR!Connected Health Care, a leading recruitment and staffing firm, is proud to partner with a state-of-the-art Skilled Nursing Facility (SNF) and Rehabilitation center located near Ontario, OR. We are currently seeking dedicated and compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our client's dynamic team. This is an excellent opportunity to make a difference in the lives of patients while working in a supportive and collaborative environment.
Job Description:
  • Provide direct patient care, including assessment, planning, implementation, and evaluation, in a skilled nursing and rehabilitation setting
  • Administer medications and treatments as prescribed by healthcare providers
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Monitor patient progress and report any changes in condition to the appropriate healthcare professionals
  • Ensure compliance with regulatory standards and facility policies
Education And Certification Requirements:
  • Current RN or LPN licensure in the state of Idaho
  • Graduation from an accredited nursing program
  • BLS certification (required for RNs; LPNs may be required to obtain)
  • Previous experience in a skilled nursing or rehabilitation setting preferred
