Our home care agency is seeking seasoned individuals who have worked as Managers in a Home Care Agency setting to add to our amazing management team. Responsibilities include but not limited to:

Scheduling

Building relationships with clients and caregivers and referral sources.

A plus if they have experience in the DSS Acquired Brain Injury (ABI) field; managing/coordinating services for individuals on the ABI Waiver.

Writing and executing Care Plans

Ensuring services authorized in plan of care are in place and addressing clients’ functional and environmental needs

Managing caregivers to ensure excellent customer service

Making home visits as needed.

Handles client or provider concerns or requests regarding services, billing, or other needs, assuring excellent customer service.

Keeps abreast with all home and community care rules and regulations; ensures that services and activities adhere to program requirements.

Attends trainings and keeps abreast of issues and information necessary to perform work duties.

Records all client related activity ; maintains up to date client files; uses client database software program to enter all client data into system.
