Job Description

Constellation Health Services is seeking a caring and compassionate Physical Therapist (PT) to join our team. As a Home Health Physical Therapist (PT) you will be the patient’s partner throughout their journey of restoring motion and ensuring that the patient will function at their personal best. You will maximize the patient’s quality of life and movement potential.

Responsibilities:

• Provide exceptional physical therapy services to patients by assessing and understanding the care plan.
• Evaluate patient's functional status and home environment for hazards or barriers.
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
• Maintain and process all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
• Submits all clinical documentation, time records and other information in the required time frame.

Qualifications:

• Licensed Physical Therapist (PT) in the state of employment.

• 1 year of clinical and home care experience is preferred.
• Graduate degree or greater in physical therapy.
• Oasis experience strongly desired.
• Have great communication and problem-solving skills.
• Have a valid driver’s license, reliable transportation and willing to cover an assigned territory.
• Experience with EMRs and utilizing a tablet and/or computer proficiently.
• Ability to work independently with minimal supervision.
• Some rotating weekend days required.
• Willing to work one (1) winter holiday and one (1) summer holiday per year.
