• We are seeking a compassionate and dedicated private duty LPN or RN to provide high quality nursing care within scope of practice to a patient requiring professional nursing services in accordance with physician’s order and in compliance with company policies/procedures, state and federal regulations. This role involves providing one-on-one in-home care to a patient requiring g-tube management and who is trach/vent dependent.

To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including, but not limited to:

• Implement physician orders in a timely, complete and accurate manner.
• Ensure the safety of the treatments, procedures, patient and environment at all times.
• Provide direct nursing care to patient as prescribed by physician.
• Follow emergency procedures when necessary.
• Maintain appropriate documentation/records of care provided to patient and any communication with the patients’ physician.
• Comply with state and federal regulations.
• Notify the physician and other personnel of change in patient's condition.
• Ensure confidentiality of patient health information per agency policy.

Experience: One (1) year experience as a Licensed Practical Nurse (LPN) or RN preferred

Licensing/Certification/Education Requirements:
Graduate of a Licensed Practical Nursing (LPN) Program or RN Program
Current Licensed Practical Nurse (LPN) license or Registered Nurse (RN) license
Current Red Cross/American Heart Association CPR Certification
Current State Driver’s License

Vent and tracheostomy experience highly preferred but not required

Equipment Physical Requirements:
Job will require frequent lifting of objects up to 40 pounds

Job Type: Full-time

Pay: From $24.00 per hour

Expected hours: 36 – 48 per week
