Continued Care, Inc is seeking a Registered Nurse. The ideal candidate would possess OASIS knowledge, good communication skills both written and verbal. The candidate must be organized and reliable transportation. We provide care in Allen, Auglaize and surrounding Counties. Responsibilities:

• RN Case Manager is responsible for the overall development and implementation of the POC
• Supervision and evaluation of care being provided by the HHA every 14 days with written documentation to substantiate visit.
• Supervision and evaluation of LPN who provides care to assigned CM patients at least every 30 days with written documentation to substantiate visit
• Attends scheduled community based care plan meetings providing input re: pt care (MRDD, CareStar)
• Regularly updates HHA care plans as needed at least every 60 days.
• Conducts SOC/ROC and recertification assessments on assigned patients
• Provides clinical oversight for problems with assigned patients as requested by LPN
• Cooperates with other agencies caring for assigned patient to provide continuity of care and implement a comprehensive care plan.
• Bi-weekly case management meeting with assigned LPN and clinical director to review overall related patient care
• Routine SN visits with assigned patients with focus on higher acuity or more difficult to manage patients
• Provides educational training regarding revising care needs; specific treatments related to patient ie: wound care, trach care, etc.
• Regular meetings with Clinical Director to provide updates on changes in patient care needs to overall patient condition
• Provide nursing services, treatments, or other procedures requiring substantial specialized skill as determined/ordered by physician
• Will function as on-call nurse as per rotational schedule
• May act as interim clinical supervisor in absence of clinical director.
